Custom domain for my forms is not working

Hi, this is the for URL.

https://form.jotform.com/72134726784159

this is my domain: myballetbelle.com

so this should work, right? ==>> http://forms.myballetbelle.com/form/72134726784159

unfortunately it is not working. please see attached image for my CNAME record
