Fields can't be filled out

Follow the link below to my website, which links back to jotform.

The fields marked "How many Wall tables?" and "How many Island tables?" should have check boxes, and a countdown number of remaining tables. But nothing appears for people to fill out. But the boxes and countdown numbers appear on the JotForm website.

So its working on the standalone form. And its not working on the website.

This problem is usually fixed by using the iFrame embed method:

This method makes the form behave exactly like the standalone form.
