How do I create a template where employees can have multiple entries?

I am going to run a promo for my team with a certain vendor and want to track conversions or new business landed. 

I am not sure if I understand your query correctly. Do you mean to send the form submission to multiple recipients? If so, you may add multiple email addresses in the email settings.
