Why does the word ''OLD'' show up in the title of my form?

When you go there, the title at the top of the browser says "OLD | Unique Marketing Personality Quiz", but when I log in to edit the title the word "Old" isn't even in the title.

 

HELP!

Upon inspection of the form you kindly provided, I found that it's the form's page title that has this text, and altering that would solve your issue.
