I am having trouble converting a PDF to make a fillable jotform

    I have uploaded both of the attached files and each time I have gotten a message back that Jotform is having difficulty converting the file.  I want to embed a copy of the file on my website using jotform  I have uploaded a copy of the pdf in this message.     Are you trying to import a fillable PDF form? You can only import an Adobe Forms Central fillable PDF. 

    The other forms may not be converted successfully or they may not be imported at all.

    If you are unable to import your PDF form then I would suggest you to create an online form with our form builder. Once you create a new form you can convert it to a fillable PDF form.
