    Hello there

    Our school goes to ISO 9001 certification system.

    And version 2015

    This data allows the online registration of forms.

    I want to use JotForm.

    The form I designed

    I want to export as pdf

    But I want to export it in format

    As in

    And I want the pdf file to e-mail me.

    As I can understand, you need to have the submitted data sent as PDF, but you need to have an specific format. Do note that it's possible to customize the PDF report layout, please check this guide that will provide you more details about it: https://www.jotform.com/help/384-How-to-Customize-PDF-Submissions-Report 

    To include the PDF report on your email notifications you only need to edit your email notification, go to the advanced tab and turn on the PDF attachment option:
