Form not showing up on ipad or phone

Hi, can you help me please for some reason the form does not show up on ipad or iphone. I've tried everything I can think of.

I have checked your website and see that the form is not being displayed correctly on iPad. I notice that you are using Wix platform for your website and the form is embedded using iframe embed code. Could you try embedding the form using the direct link instead of using iframe embed code? Please refer to the guide below that can help you with adding the form to a Wix website.
