Hi,
Please note that that custom URL feature is NOT associated with the custom domain.
Here's the guide about the custom domain: https://www.jotform.com/help/144-How-to-use-Custom-Domains-with-Jotform. This is actually not a feature on JotForm, but rather a workaround using your own web hosting service.
The on in your screenshot is the custom URL feature of JotForm, here's a guide on that one: https://www.jotform.com/help/261-How-to-Create-a-Custom-URL-of-a-Form. The custom URL feature of JotForm will only allow you to change the suffix of the link, you cannot change the whole form URL and the domain.
If you have a website, I suggest you just create a web page, then embed your form there, with that you have full control on your website link. Here's how you can embed your form on a web page: https://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page.
Also, please do avoid creating multiple threads for the same question, it is best to reply on the same thread to have a better discussion and for us to assist you better.
Thank you.
