Why is the preview different from the builder?

While building the form, I have exactly what I wanted but when I preview it, the form looks different

The only difference I'm seeing are the Sublabels of the Full Name Field (Client/Business Information). Is that what you're reporting?

It's behaving this way because the form is trying to translate the page. I managed to fix it on a cloned copy of your form by removing the translation language (since there's no translation that should be done to begin with).

If you would need to setup the form in a different language, you'd have to finish the translation and localize each field upon your discretion. We have a detailed guide that would walk you through the process.
