How to align the fields on the form?

Here is my form, the alignment looks different, I want all fields to have same allignment in terms of dropdown list / textbox/etc. , can you please help?

I have checked your JotForm and see that the dropdown width is larger than text box fields. Please try setting the width of the dropdown to 570px so that they should be aligned properly.

Alternatively, you may also inject the following CSS code to the form to change the width of the drop down field.

.form-dropdown {

  width: 570px !important;

}
