How can I create a form that includes birthdate, city and country but excludes street address?

The Happimoney Movement is a glogal movement that wants to collect millions of LIKES to impress upon politicians the need to change the law on money. We need birthdate to know what age group is most interested in Happimoney.. We need the city to enable members living in the same city to contact each other.  But we don't need street addresses.  How can we leave out the street address in our form?

Use the Birth Date Picker Widget to collect birth dates from your users. Alternatively, the regular Date Field should work the same.

And for the Address Field, you can include/exclude the visible options within the address upon your discretion:
