Why has my form changed design by itself??

I used a template from a friend of mine:

The form has completely changed without me doing anything. I re-saved the template and was going to redo the form, only to find that the template automatically changes away from how it was previous as soon as it is saved onto my account.

Please help!!

You should be able to revert your form to a previously saved state using your revision history:

I am not sure what caused the initial changes, but you should be able to revert them.
