I created the form at work for job application and can use the form as a job applicant using the computer at work. However many of the fields do not show properly when I use the form with my computer at home. Are there any specific computer / setting requirements for filling up the form?

We do not have a system requirement. As long as you can open any website properly, it will also work properly with JotForm.
Try using a different internet browser and see if it is working. If the issue persists, please provide the Form ID and screenshot of the issue so we can further investigate it.
