Form is not showing on mobile website

Cant add mobile responsive widget

my form isn't showing on mobile. I added the mobile responsive widget to the two individual check/cc forms but it won't let me add it to the main form. It's grayed out and I can't see the form at all on my website from my iPhone 7. https://www.sstte.org/kol-nidre-donations.html
