Is it possible to upload my form to JotForm and add an E-signature?

I need to have 500 people fill out IRS form W9 (a very simple form).  Is it possible to upload it to jotform for them to fill-out and e-sign?  

I know it is acceptable to do this, the question is how do I get the form imported into jotform.
