I am trying to use the employee evaluation form. The form has a problem with trying to put in the date. Could you please answer me. This is my 3rd request.

We are sorry for the inconvenience caused. Unfortunately, I am not able to find any other threads with your username. If the previous threads opened without logging in, they shall be addressed by one of our support team members. However, I have now checked your JotForm and see that the Date field on the form is working normally and I am able to go to the next page without any issue. Please see the screencast below:
