    Dear JotForm Team, 

    I would like to create a simple contact form with a file upload option (PDF, Word or Excel, for example). 

    This form would have to be integrated into the Divi WP theme. 

    Which type o form will I have to choose?

    Thank you for contact us. 

    As I checked your account, you are able to create forms both in Legacy Layout which is designed as all questions in the same page and New Layout which is designed as one field per page. 

    The only differences between this two layouts are only form preview. The remain features that you can use are the same. You can use File Upload field and restrict the allowed file extensions.

    You can use our design panel to select a theme and apply to your form. In addition to that, if you want to embed your form to the Wordpress, you can use one of your form's embed codes.
