Form information is missing and conditions logic suddenly wrong

    Hello,

    Is there a problem with Jotform software going on?

    Many of my images are missing throughout the form and recently some of the conditional logic that was created is no longer working or in place.

    Also, my revision history is not making sense.


    Please help

    Are the images in the Image Radio Button are the images that are missing on your form? If so, I cloned your form (https://form.jotform.com/72765272602154) and I noticed that the image URL's are not accessible.

    I checked your conditions as well, however, I'm not sure which conditions are not working on your form. If you could please provide us more info about the conditions that are not working, that can be a great help for us to further investigate.
