Specifically, individually differentiated. (I have a database of addresses and want users to confirm or change their data)

Yes, you can prepopulate values in the form fields via the URL. You can use the Field Prepopulator App. Here's the link: http://prepopulate.jotform.io/.

If you want to do it manually, then you can refer to this guide: Prepopulating-Fields-to-Your-JotForm-via-URL-Parameters.

    Hope that helps. Thank you.

Maybe I am thinking of something different beyond pre-populating. I want to bulk upload a list of names and addresses that would appear in a drop down menu that would be dependent on a form of conditional formatting.

For instance, form item <Name>, click the drop down and see John Doe, Jane Doe, Jerry Doe.

If user selects Jane Doe, then

Next form item <address> is prepopulated with Jane Doe's address. She can then choose to edit the form or go to the next question.

    Is this possible?

We do have a widget called Spreadsheet to Form. This widget allows you to upload your spreadsheet to your form and use the values you have on the spreadsheet to pre-populate your form.

Please check this guide for more information: https://www.jotform.com/help/442-How-to-Use-the-Spreadsheet-to-Form-Widget.

Hope that helps. Let us know if you have any questions. Thank you.
