Download a PDF for the employees to be fill out

We are trying to download a PDF for new employees to be able to fill out their I-9 and W-4's is there anyway we can do this?

We also want to make a spot for people to send us their social media accounts, Facebook, instagram, youtube.... how can this be accomplished?
