Unable to see data from spreadsheet to form widget

Hi Ive created a simple form with just a spreadsheet to form widget to test it out. Unfortunately when I publish it and clicke autocomplete fields it just hangs. Thanks for any help

I checked your form (https://form.jotform.co/73130865854865) and I noticed that there are no other fields which will be prepopulated using the Spreadsheet to Form widget.
