Iyzico Integration not working.

Hi team, i have pasted a link of the form i have created to recievce a payment with iyzico below. ıt doesn't work and i wwonder why? Please give me an assist with it. Also once payment page appears (sometimes ) it appears right at the bottom of the page which gives a blank page idea unless you scroll down. is there anyway they appear right at the top of the page?
