As the payer will be you, you don't need to use any payment integrations in your form. I would like to recommend you to use the email field absolutely. Please note that collecting any bank details in a plain text isn't allowed in Jotform. Hence, you will personally need to contact freelancers who send submissions to you.

I prepared a sample form for you. You are able to check it out on this link: https://form.jotform.com/73232541723955

Feel free to clone it on your end: How to Clone an Existing Form from a URL

I would also like to mention that users can select more than one branches with the multiple choice element. However, they can only select one branch with the single choice element.

If you have any further questions, please don't hesitate to ask.

If you use PayPal or etc. payment tools in your form, your form users cannot submit the form without paying. Hence, I kindly recommend you to use the "Purchase Order" tool. It allows you to create payment forms without payment vendor integration. In other words, no actual payments will be processed.
