I am having an issue with viewing a form I created. When I view the form on my Mac Powerbook I can see the entire form when using an external monitor, but when I try to view the form on the built in monitor, the screen size is smaller and the form is resized. I have no issues with scrolling to see the whole form, but the issue is that the top of the form is cut off and I am not able to scroll up to see it (it is off the screen). I have tested using Chrome and Safari and found the same result. I am rolling out this form as a pilot for my company, and if it goes well we may consider upgrading the account. This issue is holding up our testing, as all employees use Mac Laptops and will be filling out the form without an extended monitor. I am using a plugin for page tabbing on the form.

I am able to replicate the issue you reported on my laptop and I am using Windows 7.
I run a few tests and the Theme that you are using is causing the issue. I will report this issue to our developers and we will inform you via this thread once this issue is resolved.
For now, I can only suggest you to change the Theme of your form. Here is my cloned form with Default theme: https://form.jotform.com/73324311431948

I checked this issue with my colleague that created the Theme you are using and he told me that theme is designed for minimal form, form with 1 field like the newsletter.
However, the issue can be resolved with this CSS code:
.supernova {display: block !important;}
