I have used my personal email address to test the auto-respond function on this form: https://form.jotform.com/MassArt/i-20-request-form and it is not working. I have tried several times. I have checked my junk mail and my inbox there is nothing there saying I have completed the form. Can you please assist?

I have tried sending a test submission to the form and I have received the email without any issue. Please see the screenshot below:
