Form submit button keeps saying Please Wait

Please see the screenshot. The form won't process. Keeps saying "Please Wait."  This has been tested on all browsers (IE, Chrome, Safari)

I just tested your form, and it went through just fine:

If you have embedded your form in a webpage, please try embedding your form using the Iframe code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code 

Before you embed the code, paste it in a text editor like WordPad, you will notice two part of codes, only copy and embed the Iframe part in your webpage, example:
