Create a form with an approval workflow to be approved by multiple managers

Hi, I am creating a gift declaration form, which requires approval routing.

Basically, 

Users will have to declare amount of the gift received.

From there, if it is below the threshold set, it would require Line Manager's Approval.

if it is above the threshold set, it would require 2 stages of approval;
Line Manager's Approval & then followed by Compliance Head Approval.

We have many many many Line Managers.
For Line Manager's Approval, I would need to be able to route them to an email provided in the form as a form field.

For the second stage approval (if above threshold), it is more straightforward as we only have 1 compliance head in the whole region, and have 1 designated email to route to.

The form is still in WIP, but you can perhaps take a look at it to get a jist of what I am trying to achieve!
