I was able to replicate the issue when I opened your webpage. I believe you are using a self-hosted Wordpress website.

Please try to re-embed the form using this user guide: https://www.jotform.com/help/4-Adding-Forms-to-Wordpress-org-Version.

If it is the same, please re-embed the form using the iFrame method. Here's a guide: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.

Lastly, you might need to adjust the width of the iFrame element. Here's an example:

    <iframe id="JotFormIFrame-1234567891112" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://form.jotform.com/1234567891112" frameborder="0" style="width: 800px; min-width: 100%; height:539px; border:none;" scrolling="no"> </iframe>

Hope that helps. Let us know if you need further assistance. Thank you.
