Your account was suspended due to requesting social security numbers through forms. We do allowing for collecting such details but only in very limited circumstances. For more information, please see the following thread:

If you would like, we can remove the social security number field from the form and activate your account.

Hi David,
i actually didn't have a social security request form on there. the purpose of my form was to register groups. I didn't give them an option to put a social in the space. If something looked like that, I apologize. but there's no reason for me needing to request that information on the form.
But yes, please. please restore my form and my account. Thanks

I saw the email that had the attachment with the information requested. That might of been a form I didn't through with because I did give anyone that form to fill out. I used a LR Group Registration form I created. Please let me know when you restore the account. thanks

I have also checked your account and it is currently active, the SSN field has been removed from your forms too.
