I could not understand your statement clearly but based on the Title of your thread and the submission data of the above form my understanding is you are having duplicate submissions of same user submitting form in different times.
If this is the case and you want to limit the number of times a certain user can submit to your form, kindly use the Unique submission feature for your form. Below is a guide on how to set it up:

We are using form to process event registrations and some submissions are coming in duplicate as receive confirmation now and then 10-15 minutes later, exact copy.
For one of our staff to reuse form over and over again, that is the best setting to use in order to retain info without issues?

If you want to prevent duplicate submissions, yes, applying the suggestions provided in that guide is the best way to prevent duplicates.
