Blank fields appearing on the DocuSign widget document.

    My Jotform, BBA NDA R5, is not working as it has in the past. Here are the problems:

    1) Two fields on the form, Available Funds and Credit Score, appear blank on the DocuSign final document I receive once the form has been completed and signed. This has been the case for several months.

    2) Just this morning I received a submission for the first time in a month. However, I did not receive the usual Jotform email notification with the completed form. I only received the completed Docusign form, with the two fields mentioned in (1) still blank, although it is marked with an * as a required field.

    Related to (2), when I slog into Jotform and look at submissions, the form I just received does not show up in Submissions.

    NOTE: As per an email request I received from Jotform yesterday, I went into the form settings and re-authenticated the Docusign widget.

    Any help or suggestions here will be greatly appreciated.

    Hi Paul, 

    Thanks for reaching out to Jotform Support. I cloned your form and I was able to replicate the issue of the two mentioned fields showing blank on the DocuSign widget document. So, I've coordinated with our back-end team to review this request. Rest assured, we'll circle back to this thread as soon as I receive a response from them.

    As for your other question, I've moved that here to avoid any confusion. I'll be replying there shortly.

    Hi Paul, 

    Thanks for waiting patiently. You currently don't have a PDF template set up for the DocuSign widget document. In order for the fields to be displayed as per your liking, you need to set up a PDF template. See my screenshot below:

    • On your My Forms page, hover your mouse over the form you want to edit.
    • Left-click on More and then under the Data column, click on Open PDF Editor.

    • In the Add Element menu on the left side of the page, click on the Form Fields tab.
    • Click on the form field you want to use to add it to your PDF. Any field you choose will be shown on the PDF document to be downloaded.
    • You can also remove form fields added to the PDF document by left-clicking on the field and then clicking on the Trash Can icon on the right side of the page.
