    I need to add the below table shape for a form, what is the form elements that I should choose?


    the user have to choose yes or no ,,, incase of yes the user have to choose degree between 1 to 7 in case he choose no there is no need to any check

    You could try ith Input table field:

    Where you can create table and use conditional logic to insert certain value in another field if needed.

    However, you have to choose one type of fields that will represent the table, text field, checkbox, radio button etc.

    Another way is a Configurable list widget:
