Does jotform integrate with Avimark?

Can Jot Form be integrated with Avimark to put all our consent / surgery forms directly into Avimark?

Thanks for reaching out to Jotform Support. Unfortunately, Jotform doesn't have the option to integrate directly in Avimark. But the form's source code is available if you want to embed it with your system. Here's the guide on How to Get the Full Source Code of Your Form.
