    My video and other items are not showing up when you view the forms

    I did check your form and did not find any issue with that. You have added one logo and one image in form. Both the images are being displayed correctly in your form. Please check the screenshot below:

    I was not able to locate any video file you have added in form. Can you please share the screenshot of your form where movie file is not being displayed?
