Collect signatures into an input table?

I am trying to create a meal sign in sheet where I can collect names, signatures and resource information then input it onto a department document for accounting purposes. There are 20 spaces available per sheet and I may need to use multiple sheets. I want an interface similar to the petition template so each user can fill out their own Name, Resource ID and signature and all of the users data is collectively stored on the lines of the department form. Can I do this with Jotform?

Thanks for reaching out to Jotform Support. You can use the input table for your requirements; however, there is no option to add a Signature field. You can only choose the input type options shown here:

The signature field needs to be added individually outside the input table field.

Would there be a way to have the employees sign in with a name and signature box and have it applied to the lines of the PDF? When I use multiple boxes, it gives the users the option to fill in all of the inputs at the beginning. I want to be able to assign 1 line to 1 employee and signature, and repeat this for 20 employees. The employees change so I couldn't pre-fill in any boxes.

Yes, you can use regular form fields, then map them to your original PDF using Smart PDF Forms. Let me walk you through it:

• Click the Create Form button on your My Forms page. 
• Choose Import Form and then Import PDF Form.
• Now, upload your PDF file.

• Add the necessary fields and customize them using the Add Form Element menu on the left side of the page.
