PDF: Textual content is not showing

I created a form that requires a signature, the signature comes through but the verbiage above the signature is not showing up in the pdf. I need it to show as its a legal document.

I need everything on the forms to show up is what I'm asking - nd if they come in .pdf even better.

You can show the headers and text in your PDF by going to your submissions page and enabling the option on the submission filter (gear icon).
