Why does my form say "Please Run The Wizard"?

My form says "Please Run The Wizard" randomly in the middle of my form.  Can you please tell me why and how I can remove it?

I have just checked your form and it seems like you were able to remove the problematic element from your form:

No.  I just moved it to the bottom of the page.  Can you please take a look and fix it?

I just removed the element from your form and cleared the forms cache, please check and confirm that you do not see the text there anymore.
