Embedded forms: Two different forms on the same page, and disable after submit?

Hi,

I'd like to have two different forms on the same page.  It seems to work fine until either of the forms is submitted then they don't reload.

I would prefer to restrict the forms to 1 submission only and then perhaps disappear or disable somehow.

Is this possible?

1) Can you please tell me if you want to embed two forms in a webpage?  Or do you want two forms in a third form?

2) Can you also supply the Form ID's - Where-to-Find-My-Form-URL
