Can I have terms and conditions widget with two available options?

    HI,

    I am using Jotform for my research study for my dissertation work. I need a consent form for my participants. I have looked at the terms and agreements widget and those do not meet my needs.

    I need a form like the terms and agreements widget but with 2 check boxes: I agree and I do not agree. If the participant selects the I do not agree, I need it to be able to go to the thank you page (using conditional logic).

    Is there a way to do this? I do not want the Agree/Do not agree to be included as a question on the surveys reports.

    The terms and conditions widgets will have one option only since if the users do not select the option that means that they do not agree the terms, now with the terms and conditions widgets we already have you can set up conditions, if the check box is empty it means the users  do not agree the terms and you may do the redirection to the thank you page based on that.

    Now, if you require to have both options then you will need to display the text using a text element and the options using a check box or radio button field:

    The configure the conditions based on that as well. 

    Also, all the fields can be excluded from reports, while configuring the report you will be able to select the fields you need to include only, here is an example: https://www.jotform.com/help/101-How-to-Create-an-Excel-Report
