Upon inspecting the form, I see it has Form Calculation widgets that have equations which, based on what I have observed, is creating the conflict shown in the image below.
How-to-get-a-console-report-from-most-common-browsers
I haven't found yet which particular Form Calculation widgets are really creating the conflict, but I see several widgets having the same setup as the one shown in the image below.
The Form Calculation widget is designed to perform a calculation with numbers and not string (letters or texts). — How-to-Perform-Form-Calculation-Using-a-Widget
If you are looking to manipulate texts (strings) to insert it into another field in your form, I suggest that you refer to this guide — How-to-Manipulate-Text-String-with-Conditions-and-Calculation
I also suggest that you remove the equation on each of the Form Calculation widgets in your form first, just like what I did on this clone version of your form —
and then just set it up again following the guides I gave you above.
Also, you can tell us how would you like the calculations in your form to work and we will try our best to help you set it up.
If you have questions, please feel free to let us know.
