Can I use JotForm to create a waiver form and collect a signature?

    Hello, Bruce. Yes, you can create a waiver form. You can either make use of the text element field where you can put the description of your waiver.

    Thanks. I did try the text element field though could not get that text to be included in the email responses as part of the attached PDF. Is this possible? Also, your link to the scrollable text seems to be broken as it results in “application not found.”
