Can I merge fields?

Looking to find out if i can merge several fields so I can take a users name, date of birth, time etc. all on one page?

If this is the case, can I also regenerate that form multiple times if the user responds to a question.

For example - A manager fills in a team members details, once that is complete they have the option to repeat the process for a second team member or proceed
