Can I create a PDF form?

I have a PDF file that we created years ago that had a lot of "if this/ than that" in it...

It seems that it has a few bugs in it occasionally.

JotForm seems to be the way to go - is it possible to have a JotForm accessible off line by chance?

With JotForm you can create a PDF form; however, there are some limitations such as the conditional logic feature, widgets and some other features that work well with the web version of the form.
