My understanding is that you want to delete a cloned form in your account. In My Forms page, select the form and then click the "Delete" button.

Are you referring to this form (https://www.jotformpro.com/form/50695856200962)?
I can see that this form have 3 email notifications. 2 of them has the title "Clone of Emergency Medica Supply Request". You can remove the "Clone of" text by clicking the "Edit" icon. Here's a screenshot:
