Strange problem with my form, then stopped appearing all together

    Hi Guys - good morning.

    I've tested my form three times over the last 4 weeks or so and it worked fine the first two times.  The third time I got a message saying I couldn't submit the form because it had already been submitted from that address... the thing is - while I had that setting activated - I was testing from different locations. 

    So "fine," I went back and deleted the form this morning, changed the design to a little bit simpler look and didn't use the IP detection option.  Published the code just like the last time and copy & pasted same on contact page (http://www.splendidfoods.com/Contact_Us.php)... and now nothing shows up.  What kind of idiot am I?

    Any words of wisdom and experience?

    When I tried to access the URL you have provided it is saying File Not Found. Maybe you have deleted the file from your server?

    Also I have checked your account and found your form: http://www.jotform.us/form/80462929684166

    Note: You can create unique submissions with cookies and IP. If you enable Cookies then no matter if you change the location the cookie is stored on your browser. And even if you change the IP it wouldn't matter.

     The file is up and showing on my end.  The host (goDaddy) shows the file listed and active.  Still no script showing on the page.  This is a new version, I deleted the old form and created a simple one for display and use. I tested from different computers in different buildings on the same day so the form should have worked.  Hmmm.

    I did further checks on the URL you provided and noticed that you have the error "/ Script to open and close sidebar function w3_open() { document.getElementById("mySidebar").style.display = "block"; } function w3_close() { document.getElementById("mySidebar").style.display = "none"; }" at the bottom of the page.

    This may be the reason your form is not displaying. Probably some code is broken when you did the form embedding again.

    What I would recommend is you check on where and how you have embedded your form to the website.
