Output is email confirmation/how to have verbiage appear

The goal of my form is to have an internal employee fill it out and the output will be the email sent to the client.  As a result, I would like for my conditions to determine if a line of text is inserted into the email confirmation in addition to the fields answered.  Is that possible?

In the sample attached, I want to select #1 and have the verbiage in #2 appear on the confirmation email.

You need to manually add the Text field in the Email content. Here's a guide: https://www.jotform.com/help/430-How-to-Edit-the-Email-Template-for-Notifications-and-Autoresponders

After that, you need to create multiple notification emails with different Email content and use a condition to send email based on user's answer. Here's a guide: https://www.jotform.com/help/167-How-to-Send-Email-Based-on-User-s-Answer
