Our current plan is the free plan. We’ve had only 2 applicants complete the form and submit payment so far. Why would JotForm disable our form? I thought the free plan allows for up to 100 form submissions? Please provide any insight. We want to resolve this issue ASAP.

On the FREE plan you are limited to one payment submission per month. Payment submissions are different than regular submissions. They basically occur when you take a payment.

The solution is to either wait until the first day of next month (when the counter resets). Or you can upgrade to any paid plan with higher payment limits.
