I am trying to get this to show up on the bottom of the submitted forms:
FOR OFFICE USE ONLY:
Date rcvd:___/___/___ Date changes made: ___/___/___ Changes made by: _____
Changed on: roster ___ in binder ___ in Campwise ___ $10 fee charged to CC ____
The person filling out the form will not enter any info into these fields so it is not showing up when they submit their form. Is there a way to still have it show up so we can use it internally once we receive their form?

This is possible only when your notification is sending pdf document. to make this work you will need to disable "Autohide empty fields" as shown below.
Kindly note that this will only display in PDF document attached to the notification.
