Forms are disabled due to HIPAA

    Jotform,

    We have 2 forms that are inaccessible and are saying we need to upgrade for HIPPA. The title of these forms appears to be misleading as they do not contain any personal medical information protected under HIPPA. Our forms ARE JUST TO COLLECT NAME, EMAIL, PHONE NUMBER, AND ADDRESS

    Please unlock our forms and we will adjust the titles to prevent this misunderstanding.

    Hi William & Michael,

    Thanks for reaching out to Jotform Support. I checked your account and I don't see the forms that were disabled due to HIPAA. It may have been corrected now, can you check to confirm?

    Although I see forms that were manually disabled, you can easily enable them back from Form Builder. Let me walk you through it:

    • In the Form Builder, click on the Settings tab.
    • After that, simply change the Form Status to Enabled.
