Certain answers need to add together to come up with total at the end.

I have an excel form that I converted to a PDF to make into an online form. It is a form of questions that you answer with a Y or N. The answers have a value and need to come up with a total number at the end of the form. There are also some check boxes mixed in. Is there a widget or anything that will help make this form or am I out of luck? I will attach a pic of a form completed.

Thanks for reaching out to Jotform Support. There are no specific widgets that can do all the function that you're looking for. For your custom requirement, you will need to manually add fields and create your own form. For example, you can use regular choice fields such as Single Choice, Dropdown or Multiple Choice field for the Yes or No questions. Then you turn on Use Calculation Values if you want to calculate the total. Check the guide on this page on how to turn on Use Calculation Values of a field.
